Submission of documents to complete registration
The successful candidates in the special category of persons suffering from serious illnesses who completed their online registration through the application of the Ministry of Education, Religious Affairs and Sports must, themselves or through an authorized person, submit to the Secretariat of the Department of Informatics and Telematics (daily 10.00-13.00, Omirou 9, 177 78 Tavros, 2nd floor) by Friday 15/9/2023 in person or alternatively by postal dispatch by registered letter with proof of delivery the following documents in order to complete their registration.
-Copy of the electronic registration application from the platform of the Ministry of Education, Religious Affairs and Sports which bears, after its submission, a protocol number
-Legible copy of identity card or passport
-Clear photocopy of the Certificate of Graduation: school-leaving certificate or diploma or certificate from the school from which they graduated.

i) by the competent seven-member Committee of the hospital, as they are constituted each year by ministerial decision, in accordance with Joint Ministerial Decision no. Φ.151/17897/Β6/2014 (Government Gazette 358 B'), or
ii) by the Committee for the examination of objections of candidates with serious medical conditions for admission to Higher Education at a rate of 5% which is established by a joint ministerial decision, following a recommendation of the Central Health Council (KESY) of the Ministry of Health.

In case an in-person appearance is not possible, an additional Solemn Declaration must be submitted via the electronic application e-dilosi,to the Secretariat of your Department, in the content of which the following request will be stated:
"επιθυμώ
- the completion of my registration in the Department
- the sending of my personal access codes to the electronic services, to the following email address..........."
at this point you will enter your own email address to which you wish your codes to be sent.
Please note that, in the electronic application system for the declaration of responsibility, you log in with your own details (not the parent's) because in this way the identity of the person submitting the declaration is verified.
In the event that you do not have this option, you can contact a Citizens' Service Center (KEP), where you will similarly complete a solemn declaration and the authenticity of your signature will be certified. In that case, the sending of the solemn declaration is done by mail (Department of Informatics and Telematics, Omirou 9, 177 78 Tavros, Attn: Secretariat)
NOTE: In case the electronic registration was not completed for any reason, registration is carried out by submitting the application and the above supporting documents directly to the Secretariat of the Department either in person, by using electronic means (e.g. submitting the application via an email message) or by postal delivery, within an exclusive deadline of forty-five (45) days from the date of expiry of the deadline for submitting the electronic registration application. When using electronic means or postal delivery, completion of the registration requires the submission of a solemn declaration by the successful candidate applying for registration, which is issued via the website www.gov.gr or bears a physical signature with certification of its authenticity, in which the declarant will state that all the information and any supporting documents submitted are accurate and true.
